If you are returning to the Assessment questionnaire, the "Returning User" box is where you want to be. Just log in using your login name and your password. The system will take you to the place in the assessment questionnaire where you had previously stopped. If you are having trouble re-entering the system, click on the "Forgot Password?" link to recover your login information.
